PHYSIOTHERAPEUTIC APPROACH IN THE TREATMENT OF SPINAL STENOSIS

ABSTRACT: Introduction: A fracture is the rupture of a bone or cartilage when the applied force exceeds its resistance. It can range from a fissure to a complete break into multiple fragments. They are classified as open fractures, when they penetrate the skin and present a higher risk of infection, or closed fractures, when they do not pierce the skin. An example is a fracture of the right tibial plateau, which affects the knee joint formed by the femur and tibia, compromising essential movements such as flexion and extension, fundamental for walking. Post-operative care involves rest for consolidation, physiotherapy to recover movement and prevent fibrosis, highlighting the importance of the physiotherapist in rehabilitation. Objective: To describe the physiotherapeutic intervention and its results in the recovery of functionality in a patient with a tibial plateau fracture. Methodology: Patient A.M.C.P., 46 years old, male, with a closed fracture of the right tibial plateau and joint effusion after a motorcycle fall. He used external fixators for two months, without physiotherapy during this period. The patient began treatment at NIS with aquatic physiotherapy, presenting with stiffness, edema in the knee and ankle, difficulty bearing weight on the foot, and reduced range of motion (55° flexion and 8° extension). Exercises including gait, squatting, flexion, extension, abduction, and calf raises were performed, respecting the pain threshold. Passive exercises and myofascial release were also performed in the clinic, followed by progression to resistance exercises with 1kg ankle weights. Results and Discussion: After six sessions, the patient showed significant improvement in range of motion, increasing from 55° to 80° of flexion and from 8° to 2° of extension. Edema significantly reduced, gait progressed to the use of only one crutch, and there was greater stability. The positive response demonstrates the effectiveness of the treatment combined with active and progressive muscle strengthening work. Conclusion: Physiotherapeutic treatment was essential in the recovery process, allowing the patient to resume daily activities with greater autonomy, fewer limitations, and a better quality of life.
